Your First Visit
We want your first visit with us to be an excellent experience. To help make sure everything goes very smoothly for you, here are some important steps you can take.
Before Your Visit
Please print out the new patient questionnaire form, fill it out, and bring it with you on the day of your appointment.

During Your Visit
You will be greeted by the front office staff and then taken back to an exam room. Your doctor will ask you questions about your health history, current symptoms, and other related questions. You will undergo a physical examination and the doctor will speak to you about any additional exams or procedures needed. Routine first visit exams take approximately 30-45 minutes.
Returning Visit
For follow-up visits to our office, please print out the Returning Patient History Form, fill it out, and bring it with you on the day of your appointment.

Medication Renewal
Please call our office at least 7 business days before your medication runs out. Depending on the medication, we may require you to see your Doctor in the office before giving you the prescription renewal.
We require that our patients see us at least once a year in order to keep receiving medications for chronic illnesses such as acid reflux, or inflammatory bowel disease.
Authorization for Procedures
It is our responsibility to get authorization for your procedure from your insurance company. We will also make every attempt to notify you well in advance of any co-payments that you may need to pay before the procedure. We do expect the co-payment at least 72 hours before the procedure.
Medical Records
We will be happy to provide you with a copy of your medical chart, or fax the information to any physician you want. We do require a signed release of information form before releasing the information to anyone other than yourself.
